29 CFR 1910.212(a)(1):  Machine guarding was not provided to protect operators and other employees from hazards created by ingoing nip points, rotating parts, flying chips and sparks:    (a) Spendrup Fan Company, 2768 C 1/2 Road, Grand Junction, CO 81502:  On and before September 27, 2016, the employer did not ensure that lathes were properly guarded when used to smooth rough spots on parts.  The guard that protected operators from the rotating chuck was moved away in order for employees to perform this process.  On September 27, 2016 an employee operating the Birmingham KGC-56120 lathe, serial number 237, sustained amputation injuries while smoothing parts.  This condition exposed employees to amputation and other serious injuries by contacting the rotating chuck.     Spendrup Fan Company was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) which was contained in OSHA inspection number 91470, citation number 1, item number 2 and was affirmed as a final order on January 3, 2014, with respect to a workplace located at 2768 C 1/2 Road, Grand Junction, CO 81502.
