29 CFR 1915.73(b): When employees were working in the vicinity of flush manholes and other small openings of comparable size in the deck and other working surfaces, such openings were not suitably covered or guarded to a height of not less than 30 inches:   Onboard vessel ACANIA, main deck, portside:  Employees were exposed to a fall of approximately nine feet into the engine room when the access opening was not guarded to a height of not less than thirty inches.   OR IN THE ALTERNATIVE   29 CFR 1915.73(d): When employees are exposed to unguarded edges of decks, platforms, flats, and similar flat surfaces, more than 5 feet above a solid surface, the edges shall be guarded by adequate guardrails meeting the requirements of 1915.71(j)(1) and (2), unless the nature of the work in progress or the physical conditions prohibit the use or installation of such guardrails:   Onboard vessel ACANIA:  Employees were exposed to unguarded edges of the main deck at the access to the engine room.  This condition exposed them to a fall of nine feet into the engine room and the edges were not guarded by adequate guardrails meeting the requirements of 29 CFR 1915.71(j)(1).
