Advanced Finishing USA, Inc., 7401 Klier Drive, Fairview, PA  16415, Pangborn LK Table Blast Cleaning System, on or about June 27, 2013 and times prior thereto:  An employee accessing / working on the top of the Pangborn LK Table Blast Cleaning System to clean a screen was exposed to a fall hazard of 7.9 feet.  This employee was not protected by any guardrail systems and/or other fall protection devices.  IN THE ALTERNATIVE:  29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  A.)
