On or about June 27, 2013, an energy control program (lockout/tagout program) consisting of documented procedures, training, inspections/audits and lockout hardware had not been developed.  Employees performed servicing and maintenance on machines including, but not limited to, the OMNI lathes and the Haas CNC lathe.

On or about June 27, 2013, the two hand operation buttons were inadequately designed in that they could be defeated by the operator.  The operator could operate the machine by depressing one button and defeating the second button while holding the part in the danger zone.
