29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.     a. Facility - On or about June 28, 2013, the written energy control program did not include specific step by step procedures to control and lockout all energy sources for employees who performed maintenance on such equipment but not limited to conveyors, dumpers, grinders and wash lines. Employees were thereby exposed to the hazards associated with the unexpected energization of equipment.
