29 CFR 1910.332(b)(2): Employees, who were not qualified persons, were not trained in and familiar with any electrically related safety practices not specifically addressed by 29 CFR 1910.331 through 29 CFR 1910.335, but which were necessary for their safety a) Site - 1923 East Henry Ave. Tampa, Fl. For employees engaged in the trimming and the removal of a large tree on or about 06/20/13 which was located adjacent to overhead power lines. The employees were not trained in safe electrical work practices when working near power lines.

29 CFR 1910.333(c)(3)(iii): Any vehicle or mechanical equipment capable of having parts of its structure elevated near energized overhead lines was not operated so that a clearance of 10 ff. was maintained: a) Site - 1923 East Henry Ave. Tampa, Fl. - For the Nifty Aerial Lift - Model # TM50HG - Serial # 1721957 used on or about 06/20/13 to trim tree branches. On this date, the aerial lift was not kept at least 10 feet from the overhead, energized electrical power lines resulting in contact and electrical burns to the operator.
