29 CFR 1910.23(c)(1): Open-sided floor(s) or platform(s) 4 feet or more above the adjacent floor or ground level were not guarded by standard railings (or the equivalent as specified in 29 CFR 1910.23(e)(3)(i) through (v)), on all open sides: a) for employees working at 1400 Plantation Blvd in Plant City (FL) as observed on or about April 23, 2014. Employees were cleaning off the roof of the building with a blower. The roof had eave heights ranging from 11 feet to 15 feet and the employees were not guarded by standard railings or equivalent means of protection.

29 CFR 1910.67(c)(2)(v): A body belt was not worn and a lanyard attached to the boom or basket when working from an aerial lift: a) for employees working at 1400 Plantation Blvd in Plant City (FL), as observed on or about April 23, 2014. Employees were using an aerial lift to access the upper work platform to clean. Employees were raising the lift to a height of approximately 15 feet and stepping out onto the roof. Employees were not wearing a harness and a lanyard while operating the aerial lift.
