29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): (a) Jobsite: On or about June 28, three employees were working on a roof on a strip of eight townhouses and they were not protected from falling. The fall hazard was approximately 26 feet to the ground. The pitch of the roof was 8 in 12.

29 CFR 1926.503(c)(3): When the employer has reason to believe that any affected employee who has already been trained does not have the understanding and skill required by paragraph (a) of this section, the employer shall retrain each such employee: (a) Jobsite: On or about June 28, 2013, employees who were exposed to an approximate 26 foot fall hazard while working on a roof of an 8 unit townhouse, needed to be retrained so that they retain the knowledge and skill required to minimize the fall hazards while working from a roof.

(a) Jobsite: On or about June 28, 2013, three employees were using a 40 foot aluminumn extension ladder to access the roof and it had a broken bottom rung.
