29 CFR 1910.303(f)(2): Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, was not legibly marked to indicate its purpose, nor located and arranged so the purpose was evident:  (a) Electrical Panel HE1NW, main Hospital Building, Phoneix Indian Medical Center, 4212 N 16th St., Phoenix, AZ 85016:  On or about 3 July 2013, the legend fo panel HE1NW incorrectly identified disconnect switch # 2 as "Bacteriology Walk-In Box" when the actual # 2 location of the panel box was empty and blanked out, and slot # 1 was lined out on the legend when the actual # 1 slot had a 20 amp breaker fuse installed, exposing employees to hazards.
