29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports: a) Jobsite, Beulah Presbyterian Chapel located at 2500 McCrady Road, Pittsburgh, PA: On or before 07/02/13, an employee was working from a Waco fabricated scaffold and the working level was not fully planked. The employee was working from one, 19 inch wide platform. This was the only platform on the the working level. The fall hazard to the employee was 11 feet 6 inches to the ground.
