29 CFR 1926.453(b)(2)(v):A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. On or about May 12, 2021, during the restoration of the exterior bricks at St Mary's Church, located at 232 Seneca Ave NE, Warren, OH 44481, the employer did not ensure an employee working from a JLG Model 660SJ aerial lift was wearing and utilizing a body belt (harness) and lanyard attached to the basket of the aerial lift.
