OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing for likely to cause death or serious physical harm to employees in that employees were exposed to: Well site: On or about June 24, 2013, employees operating a service rig during the servicing of an oil/gas well worked around the well pumping unit. The pumping unit counter weights were left at the top of their cycle and were not secured to that position. This exposed the employees to the hazard of being struck by descending counter weights and/or their supporting structure.

The derrick emergency escape system was not installed. This exposed the employee to the hazards of fire, explosion, falling, and/or being struck by rig structure.

Two lines were damaged. This exposed the employees to the hazards of falling or being struck by a falling load. a) The sand line was kinked. b) The Geronimo line was severely kinked.

The employee had a damaged body harness that was used to provide fall protection while climbing and working from the derrick. This exposed the employee of falling from an elevation over 4 feet above the ground.
