OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:   Well site:  On or about September 3, 2015, and times prior to, employees were operating a service rig during the repair of an injection well which required an employee to work from the rig derrick tubing board.  The secondary means of emergency egress, Geronimo system, was improperly installed.  This exposed the employee to the hazards of striking the ground at an excessive speed or falling from an elevation.
