29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job-sites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them): Galvan's Framing @ 401 Interlocken Blvd., Broomfield, CO 80021: On and before June 15th, 2013, the exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Two employees were working next to the edge of the roof on a multi-family apartment complex without the use of a fall protection system. Employees were installing sheathing on the roof on the portion of the apartment complex. This condition exposed employees to a fall hazard of approximately 38 feet to the ground below.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8m) or more above lower levels were not protected by guardrail systems, safety net system, personal fall arrest system, or other approved alternative fall protection system: (a) Galvan's Framing @ 401 Interlocken Blvd, Broomfield, CO 80021: On and before June 15th, 2013, the exposing employer did not ensure that employees engaged in framing activities on a 6-in-12 pitch roof on a multi-family residential complex under construction were utilizing a fall protection system. This condition exposed employees to a fall hazard of approximately 38 feet.
