29 CFR 1926.20(b)(2): The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of job-sites, materials, and equipment to be made by a competent person (i.e., a person who is capable of identifying existing and predictable hazards in the surroundings or working conditions which are unsanitary, hazardous, or dangerous to employees, and who has the authority to take prompt corrective measures to eliminate them): Galvan's Framing at 11775 Wadsworth Blvd, Broomfield, CO 80020: On and before July 17, 2013, the exposing employer did not ensure a safety program was developed and implemented for the work environment to include regular inspections of the job-site, materials, and equipment; conducted by a competent person. Two employees were engaged in setting trusses from the 3rd story roof of a multifamily apartment building with a fall hazard of approximately 17 feet without the use of a fall protection system.

29 CFR 1926.501(b)(13): Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system, or other approved alternative fall protection system. (a) Galvan's Framing at 11775 Wadsworth Blvd, Broomfield, CO 80020: On and before July 17, 2013, the exposing employer did not ensure that employees were protected from injury, in that two employees were engaged in setting trusses from the 3rd story roof of an apartment building with a fall hazard of approximately 17 feet without the use of a fall protection system.
