29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:    a)  Superior Roofing, Inc. at 915 School Street, Glenwood Springs, CO 81601:  On or before July 1, 2013, the employer did not ensure that employees roofing the Glenwood Springs Elementary School, with a flat roof measuring 57 feet x 95 feet, were using fall protection.  The employees were covering the parapet walls, which varied in height from 1.5 feet to 3 feet high, with EPDM roofing material, and were setting flashing around the chimney, and no warning lines, personal fall arrest systems, or safety net systems were in place.  Condition exposed employees to fall hazards of up to 28 feet measured from the roof to the ground.    Superior Roofing, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent, 29 CFR 501(b)(10), which was contained in OSHA inspection number 313413403, citation number 1, item 1, issued September 22, 2009 and which became a final order on October 15, 2009, with respect to a workplace located at 4827 S. Wadsworth Way, Denver, CO 80123.

29 CFR 1910.1200(h)(3): The employee training did not include the requirements of 29 CFR 1910.1200(h)(3)(i) through (h)(3)(iv):     a)  Superior Roofing, Inc. at 915 School Street, Glenwood Springs, CO 81601:  On or before July 1, 2013, the employer did not ensure that employees roofing the Glenwood Springs Elementary School were trained on the chemical hazards associated with Carlisle Sure-Seal 90-8-30A, which contained Acetone, Naptha, Toluene, and Xylene, and Carlisle Sure-Seal HP-250 Primer, which contained Toluene and Heptane.  The employees were applying these chemicals without the use of chemical protective gloves.  Condition exposed employees to chemical hazards.
