29 CFR 1910.253(b)(5)(iii)(I): Fuel-gas was used from cylinders through torches or other devices equipped with shutoff valves without reducing the pressure through a suitable regulator attached to the cylinder valve or manifold: (a) In the maintenance shop, a Harris regulator for the oxygen cylinder on the Miller Big Blue welder was defective and was not operating properly, therefore it did not ensure suitable pressure regulation of the oxygen to the torch. The regulator indicated pressure when the valve was closed.

29 CFR 1910.304(b)(4)(ii)(B): Where connected to a branch circuit supplying two or more receptacles or outlets, a receptacle supplied a total cord- and plug-connected load in excess of the maximum specified in Table S-4: (a) In the office, a DeLonghi SafeHeat Oil-filled Radiator Heater Model EW0715M operated at 1500 watts was plugged into a 15 amp Relocatable Power Tap (RPT) rated for 12 amps. Also, plugged into the RPT was a Dell Computer operated at 275 watts and associated equipment (i.e. monitor, printer) operated at 60 watts, thereby exceeding the rated capacity of the RPT which overloaded the relocatable power tap and the receptacle.

29 CFR 1910.305(g)(1)(iv)(B): Flexible cords and/or cables were run through holes in walls, ceilings, floors or similar openings: (a) In the office conference room, a flexible (extension) cord was used to provide power to the copier located in the main office area and the cord was run through a hole in the wall. (b) In the ladies restroom, a second flexible (extension) cord was used to provide power to the heat tracer for sump pump and the cord was run through a hole in the floor.
