29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards: On or about 7/3/2013 the platform area above the receiving area to access the compactors; Employees accessing the upper mezzanine platform area in the receiving area to remove obstructions and work on the three compactors were exposed to a fall hazard of up to 12 feet 7 inches from the unguarded opening at the top of the platform.

29 CFR 1910.23(e)(1): Standard railing(s) did not consist of top rail, intermediate rail and/or posts and/or did not have a vertical height of 42 inches (106.7 cm) nominal from upper surface of top rail to floor, platform runway, or ramp level: On or about 7/3/2013 the stairway in the receiving area to access the compactors; Employees using the stairway in the receiving area to access the upper mezzanine area were exposed to a fall hazard of up to 12 feet from the missing midrail and the top rail installed at 37 from the top platform (step) of the stairway.
