(a) Warehouse, Phoenix Indian Medical Center, 4212 N 16th St., Phoenix, AZ 85016: On or about 26 June 2013, and at times prior thereto, the floor load capacity for the mezzanine level was not determined, designated or marked on a plate of approved design and securely affixed in a conspicuous place, exposing the employees to hazards.

29 CFR 1910.159(c)(2): The employer did not properly maintain the installed automatic sprinkler system: (a) Warehouse, Phoenix Indian Medical Center, 4212 N 16th St., Phoenix, AZ 85016: On or about 27 June 2013, the employer did not ensure that a main drain flow test is performed on the automatic sprinkler system annually, or that the inspector's test valve was opened at least every two years to assure that the sprinkler system operates properly, exposing the employees to hazards.
