29 CFR 1910.303(b)(2):Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) BATTERY CHARGING STATION: A metal receptacle box with knockouts, designed for permanent mounting, was provided with a flexible cord. The flexible cord was permanently wired into the electrical panel and was used as an extension cord for charging an electric Polaris all terrain vehicle (ATV).

29 CFR 1910.305(g)(2)(iii):Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) LOADING DOCK: The employer did not ensure that strain relief or other improved means of connection was provided in order to prevent transmitting tension to joints or terminal screws where the outer sheath of the charging cord to the Polaris Ranger 800 ATV had pulled away from the plug, exposing employees to electrical hazards.

29 CFR 1910.334(a)(2)(i):Portable cord and plug connected equipment and flexible cord sets (extension cords) were not visually inspected before use on any shift for external defects (such as loose parts, deformed and missing pins, or damage to outer jacket or insulation) and for evidence of possible internal damage (such as pinched or crushed outer jacket): a) LOADING DOCK: The employer did not ensure that the charging cord and plug to the Polaris Ranger 800 ATV was visually inspected before use for external defects. The cord was missing a ground pin on the plug end, exposing employees to electrical hazards.
