29 CFR 1910.1200(h)(1): The employer failed to provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment. On or about May 31, 2013, the employer did not provide onion room employees with information or training about the hazards of chlorine dioxide used in the onion disinfection system. The lack of training exposed employees to health hazards caused by respiratory, skin, or mucous membrane contact with chlorine dioxide.

29 CFR 1910.134(c)(2)(i): The employer did not determine that the voluntary use of a respirator did not in itself create a hazard for employees. On or about May 31, 2013 the employer exposed onion room employees to adverse effects of respirator use by failing to evaluate the potential hazards resulting from voluntary use of self-supplied respirators.
