29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about July 29, 2015, and at times prior thereto, employees were allowed to operate machines that were found not having guards provided, including but not limited to the following instances: 1) In the fruit room, a conveyor belt being operated without pinch point guards provided. 2) At line 7 in the fresh cut area, a machine being operated with a guard provided but not effectively preventing employees from exposure to the point-of-operation. 3) At the potato peeling machine area, the machine being operated with a guard provided but not effectively preventing employees from exposure to the point-of-operation. 4) At the trash compactor, the trash compactor being operated with an interlock gate provided but not effectively preventing employees from exposure to the point-of-operation. 5) At the celery line the conveyor system and rollers did not have a guard provided that prevented contact with the ingoing nip points and point of operation. Employees were exposed to possible amputation, fracture, and crushing hazards.

Employees were exposed struck by hazards in the event of an exploding wheel.
