On 7/03/2013, workers from Niemann Roofing repairing a Walgreens roof located at 455 Highway 61 N, Hannibal, MO were not wearing hard hats while removing debris, pallets, and shingles. The employees were picking up the debris, pallets, and shingles from the ground as another employee was dropping them from overhead from roofs edge.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On 7/03/2013, workers from Niemann Roofing repairing a Walgreens roof located at 455 Highway 61 N, Hannibal, MO were not utilizing fall protection while working near the edge of the roof. Three employees were observed on the roof of the building without fall protection, exposing them to fall to the ground below. One employee was leaning over the edge of the building dropping wooden pallets, debris and shingles to the ground below.

29 CFR 1926.503(a)(2): The employer did not assure that each employee has been trained, as necessary, by a competent person. On 7/03/2013, workers from Niemann Roofing repairing a Walgreens roof located at 455 Highway 61 N, Hannibal, MO were not utilizing fall protection while working near the edge of the roof. Three employees were observed on the roof of the building without fall protection, exposing them to fall to the ground below. One employee was leaning over the edge of the building dropping wooden pallets, debris, and shingles to the ground below. Also, the foreman, Darren Stephens, went up on the roof to install fall protection. He installed and tied off to an inadequate anchor. The anchor was a D ring held in place with four 16D nails.
