29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:    Prior to allowing employees to use the rough terrain forklift, the employer did not ensure that each operator had completed formal instruction training. Employees used the forklift to move bundles of shingles.

An employee was operating a pneumatic nail gun installing shingles and roofing material in the valley area on a commercial building and the employer did not ensure the employee wore safety glasses that met the requirements specified in American National Standards Institute, Z 87.1-2003.    NIEMANN ROOFING, INC. was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard: 29 CFR 1926.102(A)(1) which was contained in OSHA inspection number 916746,citation number 1,  item number  1(B), AND WAS AFFIRMED AS A FINAL ORDER ON 08/29/2013  with respect to a workplace located at 455 Highway 61 North, Hannibal Missouri.
