Warehouse: Palletized collapsed cardboard boxes were slanted and leaning, tiered two to four pallets high posing unstable loads and struck-by hazards. Abatement certification is required for this item in accordance with 29 CFR 1903.19(c)

Facility: On or about the month of July 2013 there was a forklift accident that resulted in an employee being struck by a forklift. Forklift driver did not receive refresher training after accident.
