29 CFR 1926.100(a):   Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:   a)  Worksite, 500 W. Penn St., Long Beach - Employee was not wearing a protective helmet and was exposed to being struck by asphalt shingles and tools while walking and working below overhead roofing work; on or about 07/10/13.   Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.501(b)(13):   Each employee engaged in residential construction activities 6 feet or more above lower levels was not protected by guardrail systems, safety net systems, or personal fall arrest systems, nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502.   a)  Worksite, 500 W. Penn St. Long Beach NY - Employees were patching asphalt shingles on the roof of a residential home.  An employee was cleaning up debris on the edge of the roof and was not protected from falling approximately 20ft to the ground below; on or about 07/10/13.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.

29 CFR 1926.1053(b)(22):   Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:   a)  Worksite, 500 W. Penn St., Long Beach NY - Employee was carrying a garbage can filled with construction debris down an extension ladder.  The employee was exposed to losing his balance and falling up to 20ft to the ground below; on or about 7/10/13.  Note:  The employer is required to submit abatement certification for this item in accordance with 29 CFR 1903.19.
