29 CFR 1910.215(a)(4): Grinding machinery was not used with work rest(s) to support offhand grinding work:   The employer does not ensure that work rests are used on all offhand grinding machines to prevent employee injury. This violation occurred on or about July 09, 2013, in the fabrication area where employees were exposed to struck-by hazards when operating a Dayton 8 inch bench grinder without a work rest.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all abrasive wheel machinery are used and maintained in accordance with 29 CFR 1910.215.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:   The employer does not ensure that on all offhand grinding machines a tongue guard is installed and adjusted to one fourth inch or less from the abrasive wheel or that the distance between the wheel and the end of the safety guard peripheral member at the top of the abrasive wheel does not exceed one fourth inch to prevent employee injury. This violation occurred on or about July 09, 2013, in the fabrication area where employees were exposed to struck-by hazards when operating a Dayton 8 inch bench grinder without a tongue guard to close the gap between the grinding wheel and the safety guard peripheral member at the top of the abrasive wheel.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all abrasive wheel machinery are used and maintained in accordance with 29 CFR 1910.215.

29 CFR 1910.305(b)(1)(ii): Unused openings in boxes, cabinets, or fittings were not effectively closed:   The employer does not ensure all unused openings in electrical boxes are effectively closed to prevent employee injury. This violation occurred on or about July 09, 2013, on the north side of the fabrication area where employees were exposed to electrical hazards while turning on and off circuit breakers at a breaker box without all unused openings in the box being effectively closed.  Pursuant to 29 C.F.R. 1903.19, within 10 (ten) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that the unused openings on electrical boxes, cabinets and fittings are effectively closed.

29 CFR 1910.178(l)(2)(ii): The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace:  The employer does not ensure that employees required to operate forklifts are trained and evaluated to ensure they can operate them safely. This violation occurred on or about July 09, 2013, and at times prior thereto, when employees in the facility were required to operate forklifts to move material without having been trained and evaluated to ensure they could do so safely.      Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that they provide the employees training consist of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.
