29 CFR 1910.252(a)(2)(xiii)(C): The employer did not recognize its responsibility for the safe usage of cutting or welding equipment, and did not insist that cutters or welders and their supervisors were suitably trained in the safe operation of their equipment and the safe use of the process: A.) Accudyn Products, Inc., 2400 Yoder Drive, Erie, PA. 16506, on or about October 30, 2013 and time prior thereto: Maintenance personnel did not receive training on welding / cutting safety topics related to cylinder use, storage, handling and hot work fire safety. Personnel performed oxygen fuel cutting for general maintenance tasks.
