Accudyn Products, Inc., 2400 Yoder Drive, Erie, PA 16506 on or about July 20, 2016 and times prior thereto: Employees were performing work activities that included, but were not limited to, removing hardened plastic that had bridged over in the plastic injection molding machine. During the work activity, molten plastic expelled out of the infeed of the machine and splashed onto his arms resulting in burns. The employee was not wearing protective clothing to prevent burns to the arms and torso.

Lockout / tagout is performed during operations that involve troubleshooting of the machines, and the servicing and maintenance of the temposonic rods. Periodic inspections were not performed with authorized employees.
