29 CFR 1926.451(c)(2)(iv): Front-end loaders and similar pieces of equipment shall not be used to support scaffold platforms unless they have been specifically designed by the manufacturer for such use. (a) Cottage 13 - On or about July 11, 2013 employee was observed installing shingles while working from a wood platform supported by a GEHL RSG-42 Telescopic forklift which is not designed to lift employees. The employee was exposed to a fall hazard of 11 feet 06 inches.

(a) Cottage 13 - On or about July 11, 2013 an employee was working on a wooden pallet that was not secured to a GEHL RSG-42 Telescopic forklift. The employee was exposed to a fall height of 11 feet 06 inches.

29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet (3.1 m) above a lower level shall be protected from falling to that lower level. Paragraphs (g)(1)(i) through (vii) of this section establish the types of fall protection to be provided to the employees on each type of scaffold. Paragraph (g)(2) of this section addresses fall protection for scaffold erectors and dismantlers. Note to paragraph (g)(1): The fall protection requirements for employees installing suspension scaffold support systems on floors, roofs, and other elevated surfaces are set forth in subpart M of this part. (a) Cottage 13 - On or about July 11, 2013 an employee was working from a wooden pallet attached to a GEHL RSG-42 telescopic forklift without guardrail system or personal fall arrest system. The employee was exposed to a fall height of 11 feet 06 inches.
