29 CFR 1910.146(c)(6): When there were changes in the use or configuration of a non-permit confined space that could increase the hazards to entrants, the employer did not reevaluate that space and, when necessary, reclassify it as a permit-required confined space: (a) Tube welding operations, the installation of sealer plates and end baffles changed the interior of the tube non-permit required confined spaces such that natural ventilation and egress were impeded. The written confined space policy and working procedures did not address the use and specific placement of air venturi ventilators and the periodic removal of wire welding leads to lessen the potential for accidental buildup of shielding gas in the spaces. The policy did not address the potential for trapezoidal and other end plates to so restrict the egress that employees would have to us their hands or contort their bodies to enter into or exit from the confined space, on or about, July 31, 2013. ABATEMENT CERTIFICATION REQUIRED
