29 CFR 1910.95(c)(1): The employer did not administer a continuing, effective hearing conservation program as described in 29 CFR 1910.95(c) through (o) whenever employee noise exposures equal or exceed an 8-hour time-weighted average sound level of 85 decibels measured on the A scale, or equivalently a dose of fifty percent:      a) Throughout the facility, on or about 7/18/13: A temporary employee working throughout the facility and exposed to noise at 87.3 decibels (dBA), based on an 8 -hour time weighted average (TWA), was not included in a hearing conservation program.    b) Bunching line, On or about 7/18/13: A bunching line operator, exhibiting a standard threshold shift and exposed to noise at  88.5 dBA, based on an 8-hr. TWA. The hearing conservation program was not effective in that the employee was not refit nor retrained on hearing protection.

29 CFR 1910.95(g)(8)(ii)(B): Unless a physician determines that the standard threshold shift is not work-related or aggravated by occupational noise exposure, the employer did not ensure, when a standard threshold shift occurs, that for employees already using hearing protectors, employees be refitted and retrained in the use of hearing protectors and provided with hearing protectors offering greater attenuation if necessary:  a) Bunching line, on or about 7/25/13: A bunching line employee was exposed to noise at 88.5 dBA, based on an 8-hr. TWA during the 450 minute sampling period. No refitting and retraining on hearing protection was provided after employee was diagnosed with an standard threshold shift.

The employee was working under the direction of OWL Wire and Cable Inc.    b) Multiwire 15 area, on or about 7/18/13: Training on hand protection was not provided for an employee on the multiwire line working with lubricants containing skin irritants such as ethanolamine. The employee was not wearing gloves of sufficient length to protect against skin irritating lubricants.
