29 CFR 1910.147(a)(2)(ii)(B): Lockout/tagout procedures were not implemented when servicing and/or maintenance was taking place during normal production operations when an employee was required to place any part of his/her body into an area on a machine or piece of equipment's point of operation. a. In the production area, on or about 3/5/18: An employee was performing maintenance on a Kinrei double twist bunching machine. The maintenance required the employee to place their body over the cradle which is part of the point of operation. The machine was not locked or tagged out. Abatement certification must be submitted for this item.
