MaMa Rosa's LLC located in Sidney, Ohio:  On or about July 12, 2013, the employer did not ensure the process hazard analysis addressed human factors such as employees not being able to identify certain valves mentioned in the operating procedures because the valve identification tags had fallen off the control valves.

The contractor's employees were working around ammonia refrigeration equipment and the relief header to the ammonia system while performing roofing activities.

MaMa Rosa's LLC located in Sidney, Ohio:  On or about July 12, 2013, the employer did not ensure written procedures were developed which describe how to perform internal and external inspections of ammonia system control valves.     c. MaMa Rosa's LLC located in Sidney, Ohio:  On or about July 12, 2013, the employer did not ensure written procedures were developed which describe how to perform inspections of ammonia system strainers.     d.
