Employees performing daily checks and maintenance of the 50 ton Euclid earthmoving truck were exposed to a fall of approximately 8 feet to the ground below and the employer did not ensure adequate fall protection equipment was provided. A) Employees performing work along the perimeter of the platform adjacent to the Product Feed #8 Conveyor at approximately 40 feet from the ground below are exposed to fall hazards in that a gap of approximately 14 inches was left between the toprail and the head drive roller pulley guarding. B) Employees performing work on the QC Screener elevated platform approximately 15 feet from the ground below were exposed to fall hazards and the employer failed to ensure midrails were provided for standard guardrailing and toprails at 37 inches were adjusted to a vertical height of 42 inches from the platform floor. Employees performing service and maintenance operations were exposed to amputation and the employer failed to establish procedures for the application of energy control and covered the following elements: A) All energy isolating devices that were needed to control the energy to the machine or equipment were not physically located and operated in such a manner as to isolate the machine or equipment from the energy source at the Wet Plant and Dry Plant. B) Allowed control board circuitry switches to be used in lieu of lockout devices and tagout devices for controlling energy at the Dry Plant and Wet Plant. C) Prior to starting work on machines or equipment that had been locked out, the authorized employee did not verify that isolation and deenergization of the machine or equipment had been accomplished.
