29 CFR 1926.404(f)(6):     The path to ground from circuits, equipment, or enclosures was not permanent and continuous:     (a) At the worksite - A Hilti TE7 Rotary Hammer used to drill holes in concrete was attached to an extension cord that did not have a ground prong; on or about 03/02/12.     Pane Stone Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.404(f)(6), which was contained in OSHA inspection number 280142, citation number 01, item number 02 and was affirmed as a final order on 07/03/13, with respect to a workplace located at 39-16 College Point Blvd.

29 CFR 1926.405(b)(2):     All pull boxes, junction boxes, and fittings were not provided with covers:    (a) At the worksite - A service panel used to power equipment at the site had an open slot for a circuit breaker that was not covered;  07/09/13.      The Pane Stone Construction was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.405(b)(2), which was contained in OSHA inspection number 280142, citation number 01, item number 03 and was affirmed as a final order on 07/03/13, with respect to a workplace located at 39-16 College Point Blvd.
