29 CFR 1910.119(h)(3)(ii): The contract employer did not assure that each contract employee was instructed in the known potential fire, explosion, or toxic release hazards related to his/her job and the process, and the applicable provisions of the emergency action plan. a. Cotterman & Cotterman Inc. jobsite located in Sidney, Ohio: On or about July 12, 2013, the employer did not ensure the employees performing roofing activities were provided with information and training on the toxic and fire hazards associated with the release of ammonia from the ammonia refrigeration system. Employees were performing roofing activities directly adjacent to the relief system vent stack. b. Cotterman & Cotterman Inc. jobsite located in Sidney, Ohio: On or about July 12, 2013, the employer did not ensure the employees performing roofing activities were provided with information and training on the company's emergency action plan. In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence, or written records.
