29 CFR 1926.502(d)(15): Anchorages used for attachment of personal fall arrest equipment shall be independent of any anchorage being used to support or suspend platforms and capable of supporting at least 5,000 pounds (22.2 kN) per employee attached, or shall be designed, installed, and used as follows:  Or in the alternative,  29 CFR 926.502(d)(15)(i): as part of a complete personal fall arrest system which maintains a safety factor of at least two.  a) Cotterman & Company, Inc, at a jobsite located at 250 North Third Street, Columbus, Ohio 43215: On and/or preceding July 16, 2020, the employer did not ensure employees were protected from falls by the use of anchorages capable of supporting at least 5,000 pounds or a safety factor of at least two. Employees used sheet metal brackets of an air handling unit to attach a vertical lifeline and a retractable lifeline, thereby exposed to fall hazards of up to 16 feet 9 inches.

29 CFR1926.502(d)(16): Personal fall arrest systems, when stopping a fall, shall:  29 CFR 1926.502(d)(16)(iii): be rigged such that an employee can neither free fall more than 6 feet (1.8 m), nor contact any lower level.  a) Cotterman & Company, Inc, at a jobsite located at 250 North Third Street, Columbus, Ohio 43215: On and/or preceding July 16, 2020, the employer did not ensure employees were protected from falls while engaged in commercial roofing activities. An employee was preparing the roof membrane at the edge of the roof while the rope grab on his vertical lifeline was not adjusted to prevent a fall of less than 6 feet, thereby exposed to fall hazards of up to 16 feet 9 inches.

29 CFR 1926.501(b)(10): "Roofing work on Low-slope roofs." Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system. Or, on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system] is permitted.  a) Cotterman & Company, Inc, at a jobsite located at 250 North Third Street, Columbus, Ohio 43215: On and/or preceding July 16, 2020, the employer did not ensure employees were protected from falls while engaged in commercial roofing activities. Employees were installing coping to the edge of the roof while not protected from falls, thereby exposed to fall hazards of up to 16 feet 9 inches.  Cotterman & Cotterman, Inc. was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1076435, citation number 1, item number 1 and was affirmed as a final order on March 07, 2016, with respect to a workplace located at 1219 Us 35 West Eaton, OH 45320.
