29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered: Location: 10-20 Burlington Mall Road, Burlington, MA On or about 7/13/13, workers were exposed to fall hazards of greater than 25-feet in the following instances; 1. A worker window washing from a boatswains chair only had a working line and no safety line. 2. Workers walking and working at the roof-top edge without fall protection. JC & RG CORP. DBA CRYSTAL BRIGHT WINDOW CLEANING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1910.132(A), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 314558214, CITATION NUMBER 1, ITEM 1, AND WAS AFFIRMED AS A FINAL ORDER DATE OF AUGUST 3, 2010, WITH RESPECT TO A WORKPLACE LOCATED AT 10 - 20 BURLINGTON MALL ROAD, BURLINGTON, MA.
