Citation 1, Item 2: OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees:  On or about 3/28/14, a worker moved the Fitch roof roller scaffold horizontally while a worker was suspended from it on a boatswains chair, which exposed the workers to fall hazards of greater than 80-feet.  3. Citation 2, Item 1: 29 CFR 1910.132(c):  Design. All personal protective equipment shall be of safe design and construction for the work to be performed.  On or about 3/28/14, a window cleaning worker did not have a safety line and was exposed to fall hazards of greater than 80-feet while suspended from a boatswains chair.  4. Citation 2, Item 2: 29 CFR 1910.132(a): Protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered:  On or about 3/28/14, workers were exposed to fall hazards of greater than 80-feet in the following instances;  1. Workers walking and working along the roof-top edge without fall protection.
