29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:     (a) Facility, 4026 Monroeville Boulevard, Monroeville, PA:  On or about 07/16/13 the employer did not establish an energy control program that included control procedures, employee training and periodic inspections.  Employees perform maintenance on equipment without utilizing energy controls and are exposed to energy sources such as, but not limited to: electrical, water and steam.

29 CFR 1910.303(g)(2)(i): Except as elsewhere required or permitted by Subpart S of Part 1910, live parts of electric equipment operating  at 50 volts or more were not guarded against accidental contact by use of approved cabinets or other forms of approved enclosures or by any of the means identified in paragraphs (A), (B), (C), and (D) of 29 CFR 1910.303(g)(2)(i):     (a) Facility, 4026 Monroeville Boulevard, Monroeville, PA:  On or about 07/16/13 the control button on the Wascoclean Donini dry cleaner did not function and employees open the cabinet and use a metal tool to depress the on/off switch.  The switch, operating at 220 volts, has the copper connections uncovered exposing employees to potential electrical shock hazards when performing this operation.

(b) Facility, 4026 Monroeville Boulevard, Monroeville, PA:  On or about 07/16/13 no cover was in place on the junction box located the 2nd Pants Ajax press.   (c) Facility, 4026 Monroeville Boulevard, Monroeville, PA:  On or about 07/16/13 no cover was in place on the junction box located near the steam valve for the drape press.
