29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: A) Wauseon Machine and Manufacturing, Inc., located at 635 Enterprise Ave., Wauseon, OH 43567: On or about July 16, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating chuck of the Mazak lathe machine # 159 (Model: Quick Turn 15N; Serial # 113533). Employees were exposed to struck-by and amputation hazards. B) Wauseon Machine and Manufacturing, Inc., located at 635 Enterprise Ave., Wauseon, OH 43567: On or about July 16, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating chuck of the Mazak lathe machine # 181 (Model: Quick Turn 250; Serial # 148744). Employees were exposed to struck-by and amputation hazards. C) Wauseon Machine and Manufacturing, Inc., located at 635 Enterprise Ave., Wauseon, OH 43567: On or about July 16, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating chuck of the Mazak lathe machine # 235 (Model: Quick Turn 15N; Serial # 110646). Employees were exposed to struck-by and amputation hazards. D) Wauseon Machine and Manufacturing, Inc., located at 635 Enterprise Ave., Wauseon, OH 43567: On or about July 16, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from moving parts such as, but not limited to, rotating chuck of the Mazak lathe machine # 256 (Model: Quick Turn 15N; Serial # 111391). Employees were exposed to struck-by and amputation hazards. E) Wauseon Machine and Manufacturing, Inc., located at 635 Enterprise Ave., Wauseon, OH 43567: On or about May 16, 2013, the employer failed to ensure that one or more methods of machine guarding were provided to protect employees from rotating parts such as, but not limited to, rotating spindle end of the Jet 8 Bench Grinder (Model JBG-8A, Serial # 10051337) located in Tool Room. Employees were exposed to caught-in and struck-by hazards. No additional abatement information is needed for this item.

Employees that operated the grinder were exposed to injuries from potentially jammed work and/or to struck-by hazards in a case of the wheel breakage. No additional abatement information is needed for this item.
