The knockout style receptacle outlet was not designed for portable applications in that it was connected to the end of a flexible cord and capable of being moved around the floor.

29 CFR 1910.334(a)(2)(ii): There was a defect or evidence of damage that could have exposes an employee to injury and the defective or damaged item was not removed from service and an employee was using it before repairs and tests necessary to render the equipment safe were made:     a) Insul-Board, Inc. / 2120 Colonial Ave, Erie, PA: On or about July 17, 2013 the flexible cord used as an extension to provide power to the Powerstat machine had damage at the plug end. The protective outer jacket pulled away from the plug end that was then covered in electrical tape.    b) Insul-Board, Inc. / 2120 Colonial Ave, Erie, PA: On or about July 17, 2013 the power cord going from the Powerstat machine to the hand held hot wire foam cutters was damaged at towards the center of the cord. The area was covered with electrical tape.
