OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to crush injuries, caught between and struck by hazards from industrial trucks not being equipped with restraint devices/seatbelts: Insul-Board, Inc. - On or about 9/8/17, and at times prior thereto, employees were operating a Toyota industrial fork truck that was not equipped with a seat belt or restraint device.
