29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:    12 Governor Street, East Hartford, CT jobsite:  On or about July 18, 2013, employees working on the roof were using a damaged flexible extension cord to energize power tools.  The outer jacket on the extension cord had been pulled from the fitting, exposing the interior energized wires.  The flexible extension cord also showed signs of damage caused by an overcurrent condition.
