29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: At the time of the inspection, mix room employees were adding materials into the top of the mixer with no guards to prevent exposure to rotating parts of the ribbon blades. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citation

29 CFR 1910.307(c)(2)(i): Equipment in hazardous (classified) location was not approved for the ignitable or combustible properties of the specific gas, vapor, dust, or fiber that was or could be present: At the time of the inspection, electrical equipment in the plastic mixing and extruding area was not approved for the Class II division 2 location. Dust samples were collected and the two mixtures tested were determined to be Class II dust. Equipment included but was not limited to the following; a) motors, b) fan, c) receptacles, d) junction boxes, e) disconnects. Abatement certification and abatement documentation is required for this violation. The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful. The abatement certification sheet is enclosed with the citations.
