(b) On or about 07/16/13 - Building 2, near back exit, pedestal 2-inch belt sander, the side guards were not installed. (c) On or about 07/16/13 - Building 2, Clausing Colchester horizontal lathe, a chuck guard was not installed. (d) On or about 07/16/13 - Building 2, Turn-Nado South-Bend 20-inch horizontal lathe, a chuck guard was not installed. (e) On or about 07/16/13 - Building 3, #21 Chicago green & yellow brake press, the foot pedal was not guarded.
