29 CFR 1926.451(b)(2): Except as provided in paragraphs (b)(2)(i) and (b)(2)(ii) of this section, each scaffold platform and/or walkway was not at least 18 inches (46 cm) wide: On or about June 28, 2013, an employee was exposed to the hazards of falling from an elevated work platform, while the employee stood on the top rail/guardrail, of a shop made work platform/man basket. This work platform/man basket was attached to a SkyTrak Rough Terrain Forklift, and was elevated to a height of approximately 22 feet above ground level.
