29 CFR 1910.119(f)(1)(i)(G): The employer did not develop and implement written operating procedures that provide clear instructions for safety conducting activities involved in each covered process consistent with the process safety information and did not address the steps for each operating phase including startup following a turnaround or after an emergency shutdown: a) In the Acid Oxidation Process Area (AOP), on or about July 16, 2013 - The employer did not develop and implement written operating procedures to ensure that the Bleeder Valve on a DuoPort Safety Relief Valve Manifold installed on the Neutralizer Ammonia Work Tank (T-109-3) was closed and properly seated prior to starting the plant after a maintenance shutdown.
