29 CFR 1910.219(d)(1): Pulley(s) with part(s) seven feet or less from the floor or work platform were not guarded in accordance with the requirements specified in 29 CFR 1910.219(m) and (o): At the workplace located at 17562 West Gum Road, Carthage, Missouri 64836: An employee conducting maintenance on the Series 9150 Vehicle Slide Gate Operator was exposed to pinch point and amputation hazards. The pulley assembly on the Series 9150 Vehicle Slide Gate Operator was not adequately guarded that fully enclosed the moving parts on all exposed sides.
